What should I ask in a customer problem interview?

I want to learn whether a problem is real without leading people toward my idea. Which questions produce useful evidence?

Ask about the last time the problem occurred, what they did, what it cost and who made the decision. Avoid pitching until you have heard concrete examples; behaviour is stronger evidence than polite enthusiasm.
